Appointment Requirements

You must complete your application and gather your documents before your appointment. Late arrivals, incomplete applications and improper payment methods will require rescheduling. Please come prepared. We have a checklist for you to use to be successful in your first appointment.

What to Bring

• Completed DS-11 (Download). Must be in black ink, single sided, complete and legible. • A check or Money order made payable to the US Department of State in the exact amount of your fees. Common fees are listed here. • A separate processing fee payment for the Law Library $35.00 plus unprovided copies. • Original Citizenship Document + 1 photocopy of the front and back (barcode). Examples: expired passport, an original birth certificate or a naturalization certificate. Note Birth Abstracts cannot be used in place of a Birth Certificate. See the US State Department website for information about acceptable documents. • Original photo Identification + 1 photocopy of the front and back of the ID. Examples: California driver’s license, valid or expired passport, or military ID. If you don’t have one of these see the US State Department website for information about acceptable documents. • A passport photo 2x2 that meets US State Department guidelines. You can find the requirements at the US State Department Website. • Applicants ages 16-17. Must be present and able to show parental awareness that they are applying. Payment of fees with a parents’ check meets this requirement. Where an applicant (age 16-17) does not have sufficient identification, at least one parent with valid identification is required to be present. The US State Department website has information about acceptable identification documents for 16-17 year olds. • Applicants ages 0-15. Generally, BOTH parents and applicant/child must be present with evidence of parental relationship, such as a birth certificate, and proof of the parents’ identities. Both parents also need their original ID and a photocpy of the front and back. If a parent is unable to appear, a notarized Form DS-3053 or other documentation is required. The US State Department website has information about applying for children. • You may need to provide documents in addition to those listed above such as marriage certificates, custody papers, lost passport statements, or statements of consent. The US State Department website has complete information about individual circumstances and exceptions. You will need photocopies in addition to originals.
